How to manage appointments or prescriptions for a relative.
What proxy access means
Proxy access lets you book appointments and order prescriptions for someone else.
You might do this for a partner, a parent, or a child.
How to apply
Both of you need to fill in a form. You can collect one from reception or ask us to send you one.
We will need to see photo ID for both of you.
Children
You can usually have proxy access for a child under 11. After that we review it.
Young people aged 11 and over can ask for their own access.
People who cannot make decisions
If someone cannot make decisions for themselves, we can still grant access. We will check that it is in their best interests.
