Fit notes (sick notes)
How to get a note for your employer when you are ill.
If you have been ill for 7 days or less
You do not need a note from us. You can fill in your own form. This is called self certification.
Ask your employer for form SC2, or download it from GOV.UK.
If you have been ill for more than 7 days
You may need a fit note from a doctor or nurse. Send us a request online or call us.
Tell us the dates you have been off and what is wrong. You usually will not need an appointment.
Backdated notes
A fit note cannot cover a date before we knew you were ill. Contact us as soon as you can.
Cost
Fit notes for NHS purposes are free. We charge for private certificates. Ask reception for our current fees.
