The information we routinely publish, and how to request more.

The Freedom of Information Act 2000 gives you the right to ask public bodies for information.

Our publication scheme

We follow the Information Commissioner's model publication scheme for GP practices.

This means we routinely publish information about:

  • who we are and what we do
  • what we spend and how we spend it
  • our priorities and how we are doing
  • how we make decisions
  • our policies and procedures
  • the services we offer

Making a request

Write to the Practice Manager at the surgery address, or email us. Requests must be in writing.

Tell us clearly what information you want.

We will reply within 20 working days.

What is not covered

The Act covers our NHS work only.

We cannot give you personal information about patients. If you want your own records, that is a subject access request. See our privacy notice.

Charges

Most requests are free. We may charge for photocopying or postage. We will tell you before we do.

If you are unhappy with our reply

Ask us to review it. You can then complain to the Information Commissioner's Office at ico.org.uk.
